ACCREDITATION APPLICATION FORM

Before commencing your application for accreditation, please ensure you have the required information ready to submit.
Please be advised that incomplete Applications for Accreditation cannot be saved.

  • A passport style photo, which meets the rules listed below
  • The personal information you supply is up to date as possible


PHOTO RULES

  1. Photos must be clear, sharp and in focus
  2. Photos must show both edges of the face clearly
  3. Photos must show a full front view of face and shoulders
  4. The face and shoulders must be centred within the image
  5. There must be no hair across the eyes
  6. No hats or head coverings, except for religious reasons
  7. Full facial features should be clearly visible
  8. Photo should not be covered by shadows
  9. Photo must have uniform lighting
  10. No flash reflection on the face
  11. Photo has a plain, uniform, light-coloured background
